Where questions require a rating please use the followng, 1-very poor, 2-Poor, 3-Average, 4-Good, 5-Excellant.

Please answer all of the questions (using the N/A option where applicable or by typing N/A in a text field, if you have no opinion or experience of the relevant question) and use mouse clicks to move between fields. When you have completed the questionnaire click the SUBMISSION but just once (you'll be asked to complete any questions that you may have missed).
